Sec. 48-81. - Food establishment permit and certification fees


There shall be charged the following fees for permits, certifications, and inspections by the Wichita Falls Wichita County Public Health District under Chapter 26, Article IV, Food and food vendors and establishments:

(A)  Permits:
1. Process 1 (low to moderate risk): $200.00.
2. Process 2 (high risk): $225.00
3. Process 3 (very high risk): $300
4. Temporary events: $30.00 for non-profit event; $50.00 for for-profit
5. Additional permitting $125.00 fee for each of the following activities:
A) Meat Market
B) Catering
C) Bakery
D) Snack Bar
E) Fish Market
F) Commissary
G) Produce Department
6. Seasonal permits: (valid for no longer than 6 months) $175.00
7. Grease Traps $75

(B) Certifications
1. Frozen Dessert Establishment Certification: $150.00.
2. Frozen Dessert Operator Certification : $20.00; valid for two years
3. Food Handler's Training Certificate: $20.00; valid for two years
4. Replacement Food Handler's Training Certificate: $5.00

(C) Re-inspection fees:
1. Each inspection: $75.00
2. Soft-Serve Sample fee: $25.00

(D) Plan Review fees
1. New construction: $150.00
2. Extensive remodel that requires construction: $100.00
3. Concept change that requires change of equipment: $100.00
4. Each requested site visit: $50.00

(E) Sec. 48-82. - Fees for Ambulance Franchises and Permits
1. Initial franchise application fee: $500.00
2. Annual franchise fee: $100.00
3. Annual permit fee per ambulance: $100.00

(F) Sec. 48-83. - Foster Home and Day Care Inspection fees
1. Foster home inspection fee: $50.00
2. Day care inspection fee: $100.00

(G) Sec. 48-84. - Public and Semi-Public Water-Related Activities
1. Swimming pool  and spa permits: $200.00 per pool per year
2. Manager of operation: $50.00 per manager per year

(H) Sec. 48-85. - Late fee
1. Late charge for renewal of expired division 1 permits: $25.00 for every month expired.

(I) Sec. 48-86. - Body Art fees
1. Annual Body Art Establishment Permit fee: $600.00
2. Initial Annual Body Art Operator License fee: $500.00
3. Renewal Annual Body Art Operator License fee: $100.00
4. Initial Testing fee: $100.00, which covers two attempts to pass the examination, and which amount will be credited toward the initial annual body art operator license fee.
5. Additional attempts to pass the examination: $50.00, which will not be credited toward any license or permit fees.
6. Apprenticeship fee: $100.00
7. Temporary Tattoo Artist fee: $100.00

(K) On-Site Sewage Fees (OSSF):
1. Existing, Wichita County: $150.00
2. Existing, Clay & Archer County: $150.00
3. New / Final Inspection Wichita Co.: $250.00
4. New / Final Inspection Clay & Archer Co.: $250.00
5. New / Final Inspection ATU Wichita Co.: $275.00
6. New / Final Inspection ATU Clay & Archer Co.: $275.00

(L) Lodging fees:
1. 1-10 Rooms: $100.00
2. 11-25 Rooms: $125.00
3. 26-50 Rooms: $150.00
4. 51-75 Rooms: $200.00
5. 76-100 Rooms: $225.00
6. 101-150 Rooms: $250.00
7. 151-200 Rooms: $300.00
8. 201+ Rooms: $350.00
