Community Diabetes Education Program (CDEP)

Diabetes Prevention and Management Classes

The Community Diabetes Education Program (CDEP) is for individuals 18 and up who have been diagnosed with Type 2 Diabetes, Pre-diabetes, or are at risk of developing Pre-diabetes.

Our FREE education classes are for individuals who have been diagnosed with Type 2 diabetes. They are held once a week for eight weeks and are taught by trained Health Educators using licensed Diabetes Self-Management Education (DSME) curriculum. Topics include:
  • Understanding the Human Body
  • Risk Factors
  • Blood Glucose Monitoring
  • Physical Activity
  • Nutrition
  • Complications
  • Medications
  • Coping with Diabetes

Education classes (for individuals who have been diagnosed with Type 2 diabetes):

  • Tuesday, January 15, 2019 9:00am-11:00am (English)
  • Tuesday, January 15, 2018 6pm-8pm (English)

  • Diabetes education classes in Spanish: TBD (to be determined). We are currently enrolling. Please call for more information.

Healthy Living:  Our FREE Healthy Living classes are for individuals who have been diagnosed with pre-diabetes or are at risk of developing pre-diabetes. Classes are delivered by a trained Lifestyle Coach who facilitates a CDC-approved curriculum focused on supporting lifestyle balance, healthy eating, physical activity, and motivational support.
